WHO WE AREThe In Home Support Program is a dynamic program that
provides individual supports for people with mental retardation living in the
counties of Goochland, Powhatan and Hanover.
Services are innovative, flexible and tailored to the needs of the
individual. We are a joint partnership between Goochland-Powhatan Community Services
and Hanover Community Services. WHAT WE DO
We provide one on
one service to help individuals and their families so that the individuals to
whom we provide services can stay in their own home rather than go to an
institution or a more restrictive environment.
The service each person receives is based on their needs, the number of hours
approved by Medicaid and
goals agreed on by the individual and their family. Our
focus is to train individuals to be as independent as possible so that they can
live as independently as possible. Some
of the things we do fall into the following categories: Ø
Personal Care: we help individuals with bathing, tooth brushing, toileting, eating,
hair care, etc Ø
Community Skills: we take people
into the community to provide new experiences, build relationships in the
community and be a part of the community at large through every day experiences
and volunteer opportunities. Ø Leisure Skills: We help people develop their interests and hobbies.
Ø
Activities of Daily Living:
this covers anything you would do in a day, for example, grocery shopping,
housekeeping, cooking, laundry Ø
Health Skills: we
provide education around a healthy lifestyle to include diet, exercise,
understanding medication, different aspects of a persons physical condition,
stress and anger management, etc Ø Social Skills: through training goals, we encourage individuals to interact in their community so that they can see and learn by example appropriate ways to interact and socialize with others.
WHO IS ELIGIBLE FOR
SERVICES?
Anyone who is over the age of 6, has a primary diagnosis of mental retardation and lives in Hanover, Goochland or Powhatan County is eligible to receive services. Children under age 6 with significant delays may also qualify. Our Program is funded through the Medicaid Waiver or private pay. HOW DO I GET INTO
