Same Day Access

If you are looking for either mental health, developmental disability or drug and alcohol treatment services for youth and adults, you can come in person, without prior appointment, to be screened for services called Same Day Access (SDA) assessment at either one of our two locations: Goochland location (3058 River Road West Goochland, VA 23063) and Powhatan location (3910 Old Buckingham Road Powhatan, VA 23139). Monday through Friday from 9am-3pm.

During this assessment we will provide:

  • An explanation of agency services
  • Explain diagnosis and recommended services
  • Admitted individuals will leave with a service plan and follow-up appointments and/or with referrals to appropriate community resources if that better fits the individual’s needs.

We serve those with or without insurance, regardless of ability to pay.

What should I bring to my SDA appointment?

You may be asked to provide some or all the following as part of the Same Day Access assessment:

  • Photo ID
  • Social security number
  • Insurance card
  • Proof of residence in either Goochland or Powhatan
  • Proof of household income
  • Legal guardian and custody documentation (if applicable)
  • List of all medications, dosages, and prescribing doctor
  • Name of physical health conditions
  • Name, address, and phone number for an emergency contact person
  • Name, address, and phone number for primary care physician (PCP)
  • Court Order, if required by court to complete Mental Health or Substance Use assessment

What happens when I come in for my SDA appointment?

  • You will check in with the front desk and be asked to complete paperwork and provide some basic personal registration information and a brief reason for your visit to help us better understand your main concerns and the symptoms you are seeking help with. You will be informed the intake process may take up to 90 minutes or more.
    • Your paperwork will include the DLA-20 (Daily Living Activities), brief instrument designed to measure the everyday parts of life impacted by mental illness or disability.
      • If you need any language assistance, please let staff know so arrangements can be made for a professional phone or video interpreter if needed.
  • You will be asked to wait in the waiting area while your information is being reviewed by GPCS staff.
  • After your information has been reviewed you will meet with GPCS staff who will complete a Same Day Access Assessment, where they will ask the individual about symptoms and history.
  • The GPCS staff will type the information into an electronic health record.
    • The electronic health record includes the individual’s personal information and will not be shared without your permission. More information about federal confidentiality and where exceptions are made for safety and court ordered involvement will be discussed further with a clinician.
  • Staff will talk with you about whether you or the individual you’re helping to get services appear to be eligible for GPCS services, or whether there might be another resource in the community that would be more helpful and appropriate.
    • If you or the individual seeking services do not appear to meet GPCS admission criteria, you will be given a list of other service providers and resources in the community. If you are unable to get help from these listed providers, please contact GPCS 804-556-5400 (Goochland) or 804-598-2200 (Powhatan) and we will try to help you find other resources.
  • After the assessment, the individual will receive an explanation of diagnosis and treatment recommendations which could include various services, depending on the level of care you require.
  • If you decide to pursue the GPCS services recommended, the clinician will explain the next steps on starting your journey toward healing and recovery, as well as provide the individual with an initial service plan (ISP).
  • You will leave the appointment with either referral for other community services or your next appointment with GPCS primary provider.
