Case Management Services
The case management unit provides a wide variety of services to individuals with mental illness and/or intellectual disabilities. Some of the services include:
  • Ongoing monitoring for symptoms, medication compliance and effectiveness
  • Supportive counseling
  • Collaboration with other agency programs, such as:
    • Psychiatric services,
    • Nursing services
    • Outpatient therapy
    • Psychosocial rehabilitation
    • Day support
    • Mental retardation in-home support services and mental health support services.

  • Coordination of care with family members, private providers and community programs, such as:
    • Medical doctors
    • Group homes
    • Adult living facilities
    • Other psychosocial and day support programs
    • County social services departments
    • The Social Security Administration
    • The Department of Rehabilitation
    • Other vocational providers
    • Medicaid and Medicare
    • Other insurance companies
    • The courts
    • State hospitals
    • Training facilities.


    The level of case management support a person receives is based upon his/her needs. Medicaid and the intellectual disabilities waiver pay for case management services, if certain eligibility criteria are met. Private insurance does not pay for case management, however, services are provided regardless of insurance or income on a sliding fee scale.
